After Prom Grand Finale Registration & Forms

If you are the After Prom Party Parent Representative for your high school:

Please use the Registration Form to register your high school for the After Prom Grand Finale Event.

As soon as possible, after your after prom party where you identified the students from your school that are eligible to participate in the After Prom Grand Finale Event, please submit the names of your qualifiers and alternates to RAYSAC with the Qualifier Form.

Please make copies of the Qualifier Guidelines and have each qualifier and alternate read and sign the form. The student and/or the After Prom Party Representative should bring the form the day of the event.

If any of your qualifiying students have a school sanctioned event on the day of the RAYSAC After Prom Grand Finale please arrange for another student of similar age to stand in as a proxy. Please review the Proxy Guidelines with the qualifier and the student that will stand in at the event.
